Make the Change - Ontario Example Forms

We have set up a basic demo of the web service that we have been discussing federally for Ontario's Parliament. We've got some questions about the data from Elections Ontario, but we'd appreciate testing out the postal code to lookup from the web form below:

We are hoping to set up similar tools for other provinces as soon as the data becomes available.

The first phase will take a URL string which includes a postal code and an API key that returns a machine readable multi-line CSV. Other phases include adding and combining other data sources and providing this available to members in a convenient manner that allows them to make more effective campaigns.
